If you wish to obtain a copy of a summary event report (including, if necessary, the list of stolen property) or an accident report, please submit your request in writing to the following address:

You can also submit your request to the nearest police station.
The fee is $7 (summary event report) or $14 (accident report) for a copy of a document.
We recommend that you contact your insurance company before submitting a request. For all other documents, please address your request in writing to the nearest police station in accordance with the Act Respecting Access to Documents Held by Public Bodies and the Protection of Personal Information.
Gatineau residents can request a criminal clearance check for $67 ($10 for an additional certified copy). The application form must be submitted to the Gatineau police, along with a photocopy of two pieces of photo ID (driver's licence, health insurance card, etc.).

To obtain a copy of a document held by the SPVG (other than an accident report or event report), such as an incident report, a summons or an alcohol level certificate, you must submit your request in writing and signed to:

Your request must be as detailed as possible in order to determine the nature of the documents being request, and to locate them. Also, only those requests that are signed and accompanied by a copy of an official ID (see the ID section) will be processed.
. Simply print the form, fill it out and send it to us by mail, fax or e-mail in PDF format.
You must provide your name and current full address and telephone number. Also, attach a copy of a government-issued ID (i.e. health insurance card, passport or driver's licence) that has your signature. This information will be treated confidentially, and will only be shared with officials who are authorized to handle your request.

Allow up to 20 calendar days for your request to be processed following the date it is received.
The SPVG may refuse to provide a copy of a document or to confirm its existence if it contains information that could interfere with an investigation or court process, reveal investigative techniques, jeopardize the safety of an individual or harm an individual who provided the information or is the subject of the information.
All personal information, that is to say anything that could identify an individual, is confidential, except where otherwise provided under the Act. Such information may only be disclosed by the individual who is the subject of the information. The official in charge of access to information is responsible for, among other things, protecting this information.
A pardon is a way to acknowledge that you are a person in good standing. Subject to certain conditions, individuals can apply for a pardon if they were found guilty of violating a Canadian federal law or regulation, even if they are not citizens or residents of Canada.
Application for pardon forms are available through the police. The forms are bilingual and come with a guide on the entire process.
There is a $26 fee for an application for pardon.
